Certificate formats vary, but all certificates must have the participant's name, session (Unit ___, Part ___) label along with the date of attendance.

**Please note that you can submit Part 1 and Part 2 separately.**

**TIP: It is helpful to begin the work log as soon as you start the work and track your hours as you go along rather than backtracking over time.**
Write your name and employee number on the top lines.
Write the date that you are submitting your work log.
Write your duty day hours (required!). These duty day hours would be the exact times and days that you report to your site.
Write your school name, location code, and LETRS phase on the subsequent lines.
In the table, write the exact dates that you attended the live sessions in the first two columns labeled "Day Worked" (Mon, Tue, etc) and "Dates Worked" (8/12, 8/13, etc.)
In the table, write the time your session started on that day in the third column labeled "Time In."
In the table, write the time your session ended on that day in the fourth column labeled "Time Out."
In the table, write the total number of hours for your session on that day and round that number up to the nearest .25 hour in the fifth column "Total Hours Worked"
15 minutes = .25, 30 minutes = .5, 45 minutes = .75
Ex. If you worked 2 hours and 13 minutes, then your total would be "2.25 hours."
Ex. If you worked 5 hours and 49 minutes, then your total would be "6.0 hours."
In the table, write "LETRS Unit ___, Part 1," and/or "LETRS Unit ___, Part 2," in the last column labeled "Work Performed." This should match the session you attended on that day.
Add up the total number of hours worked from the fourth column and write that number on the Total Hours Requested line.
Print and sign your name on the last two lines labeled "Employee Name Printed" and "Employee Signature" using colored ink.
Have your administrator print and sign their name on the bottom lines using colored ink.
**TIP: Save your certificates on your APS Google Drive and in a location that is on a personal account. You may need this documentation if you change school districts.**
Gather your work log and your LETRS attendance certificate(s).
Make sure the total hours on your work log do not exceed the maximum 3 hours per LETRS session.
**TIP: Make a copy of these documents and save the copied version in your records in case of timesheet issues or discrepancies.**
Put all documents in an interoffice envelope.
Fill out the envelope directory information.
Send to Teaching & Learning Department - LETRS, 530W City Center.
Participants are also welcome to drop-off their documents in-person. Electronic submissions cannot be accepted.
Do not submit your documents directly to Payroll. They will send them to us first for processing and it will delay your payment.
