PDF Editing Software
UAF offers two downloadable software options for creating fillable PDFs:
Foxit PDF Editor Pro is available for free via download at Self Service (Software Center on PC and Self Service for macOS) for UAF, SW, and UAA. For UAS users and those without Self Service on their computers, refer to the files for manual installation.
Adobe Acrobat Pro DC is available for users who fill out the UA Adobe Creative Cloud Request form (UAF/System Office | UAA).

Add new form fields and field names, if needed.
Use the top toolbar and adjust the layout with the tools in the right pane.
Save your fillable PDF document.
From scratch:
Click Tools, then select Prepare Form. Choose Create New, then click Start.
Save the PDF. Then click Add Text in the toolbar and add whatever information you want to your form.
To change existing text boxes or images, click Edit in the toolbar.
Add fillable form fields by selecting your preferred field type from the toolbar or by right-clicking anywhere in the form and choosing the Form Field tool.
Click the desired location with your cursor to place the selected field. And then set the field properties.
Once finished, save your form.
