Senior Packet
The Senior Packet
There are three parts to the Senior Packet, each of which can be found below.
Please note, the entire Senior Packet must be submitted to your counselor before any
information can be sent out to colleges.
(1) Records Release Form: This gives us permission to send out your personal information (transcript, address, and more) to colleges at your request. It
requires a student and parent/guardian signature.

(2) Questionnaire: A page of open-ended questions that help your counselor
to learn more about you and write a letter to colleges that best reflects your
strengths and accomplishments.
