As cold and flu season begins, please remind your children to wash their hands frequently and to cough and sneeze into their elbows. Practicing this at home makes it become a habit! Getting enough sleep and eating nutritious meals is helpful as well. Children with a fever should not be sent to school and should not return to school until they are fever-free without fever-reducing medication and they are eating and acting normally.
A doctor's order is required for medication (including cough drops) to be given during school hours.
Does your child get enough sleep?
The American Academy of Pediatrics (AAP) and Centers for Disease Control and Prevention (CDC) recommend the following:
Ages 3-5 years: 10-13 hours (including naps)
Age 6-12 years: 9-12 hours
Age 13-18 years: 8-10 hours

1) School physical form - physicals are required for K, 6th, 9th and new students - this can also be used for sports of all ages (Child Health Exam)
2) Lead assessment questionnaire - required for pre-K and K (Lead_LRAQ)
3) Sports physical form - can be used for sports of all ages, but CANNOT be used to meet the school physical requirement as explained in #1 (IHSA Pre-participation Examination)
4) Dental form - required for K, 2nd, 6th, 9th (Dental Exam Proof)
5) School Medication Authorization Form - needed for ANY medication (except for sunscreen and an inhaler carried by the student - see #6) (School medicine authorization form)
6) Self-Carry Med Form - used only for inhalers carried by the student(does not require physician signature) (parent agreement............)
7) Eye Doctor Examination Form - required for K and new students who have never attended an Illinois school (EyeExamReport)
