Prescription Medication
Medications that are given once or twice daily can generally be given at home; however, if your child requires a medication to be administered during school hours please note the following:
Please provide a physician/NP signed health provider medication order and parental consent form (below). All prescription medications must be in their original containers with labeling that includes your child's name, the medication name, dosage, route, and time of administration. If you have a paper prescription please also include that along with the medication.
Medications must be brought to school by an adult.
I have a small refrigerator if needed.
All medications will be returned to parents/guardians on or near the last day of school as appropriate. Please make arrangements to pick up medications. No medications will be sent home with students or stored over the summer.
New medication orders are needed for each school year.

Over-the-counter (OTC) Medication
Medicine that is purchased off the shelf is known as over-the-counter (OTC) medication. This includes products such as eye drops, cough syrup, Tylenol/acetaminophen, Motrin/ibuprophen, vitamins, supplements, cough drops, nose spray, etc.
Please note that I only have a supply of cough drops and Tums. All other medications must be supplied by you. It is helpful to have medicine such as Tylenol in school in the event your child needs it.
If you wish for your child to have an over-the-counter medication while in school, please fill out the attached OTC form and bring it to school along with the medication. You should stipulate under what conditions you want the medication given. For example: Children's Tylenol, two teaspoons for pain or headache as needed throughout the school year. No OTC medications are to be sent to school with your child and parent notes will not be accepted.
Similar to prescription medication, all medications will be returned on or near the last day of school. None will be stored over the summer and a new parental order is needed each school year.

Medications Taken At Home
Each year we request updated information about each student for the Health Office. As an extension of the medical home it is important that the school nurse receive up to date information about all medications your child takes on a daily basis. This does not include vitamins or a course of antibiotics for a short term illness.
Please download the attached letter and send it in to me if applicable. All information is kept confidential.
