SIXTH GRADE REQUIREMENTS
Beginning with the 2009-10 school year, all students entering sixth grade are required to have a current physical examination and all immunizations must be up to date. The examination must be completed within one year prior to entering sixth grade. The physical form that is required by the State of Illinois is included in the sixth grade packet and can also be obtained from the school office.
A dental form is required for all sixth grade students. The dental form that is required by the State of Illinois is included in the sixth grade packet and can also be obtained from the school office. The dental form must be completed and returned to school by May 15 of the school year.
SPORTS PHYSICALS
All seventh and eighth grade students participating in the extracurricular athletic program, including cheerleading and poms, are required to have a physical examination prior to tryouts and/or participation. Physical examinations are good for one calendar year from the date the examination was performed and must carry the athlete through the season. Physical forms are available in the school office.
Beginning with the 2009-10 school year, sixth grade students will not be required to have a sports physical PROVIDED the doctor has checked the box for INTERSCHOLASTIC SPORTS participation on the back of the sixth grade physical.
MEDICATION POLICY
Any child requiring medications while at school must have the pink “Permit for Administration of Required Medications” completed, with signatures, on file at school. This form is required for both prescription and nonprescription (over the counter) medications. Medications must be brought to the nurse’s office, appropriately labeled in the original package (over the counter) and with the pharmacy label (prescription medications).
HEALTH CONCERNS
If your child has any health concerns, please indicate them on the Yellow Emergency Form required for each student. This will allow the health office to make necessary arrangements if needed.
ILLNESS POLICY
Please notify the school office if your child will be absent due to illness.
As per District 101 policy, any child with a temperature of 100 or greater will be sent home. A child sent home with a fever should not return to school until the fever has been absent for 24 hours.
Students should stay home if they have any of the following symptoms:
Fever within the last 24 hours
Nausea, vomiting, diarrhea within the last 24 hours
Rash of an unknown origin or any known contagious rash
Pink, itchy, or crust eyes
24 hours after hospitalization
