Application Checklist

Apply to college: 3 easy steps!

Transcript Request form:  Print and return to guidance 3 weeks prior to an application deadline. Make sure your senior year schedule is finalized and your colleges are listed on BOTH your Naviance AND Common App accounts before you submit your Transcript Request google form to guidance. (Do NOT request transcripts, or other school records, through Common App or Coalition application portals.)

Academic eligibility 

YCSD Academic Eligibility Policy:  available on the YCSD athletic webpage. Appeals will only be considered two weeks after the original notice was given, unless there was a teacher error.  Status change request form link

YCSD Family Technology Support Form Help Desk: Complete and submit this form to obtain tech assistance if working remotely from home.  Visit the high school library if you are in the high school building for chrome book related questions.

RIGHT TO WITHHOLD THE RELEASE OF CERTAIN INFORMATION FROM MILITARY RECRUITERS AND INSTITUTIONS OF HIGHER LEARNING

Pursuant to the NO CHILD LEFT BEHIND ACT (NCLB), the Yorktown Central School District must disclose to military recruiters and institutions of higher learning, upon request, the names, addresses and telephone numbers of high school students. The District must also notify parents of their right and the rights of their child to request the District not release such information without prior written parental consent.

Parents wishing to exercise their option to withhold their consent to the release of the above information to military recruiters and institutions of higher learning must sign and return the attached form to the high school Guidance Center.  Please note this  form must be filled out each year your child is in the high school.  Please click on NCLB letter below.
