This form needs to be completed and uploaded with the Wish request. Information on the Wish Children are always needed when a request is submitted. This form is mandatory.

The Standard form (MAWFA) is recommended for specific Celebrity where a lot of wish details are needed. For other requests (travels) the simplified form can be used.

When a Wish Child is traveling abroad the Assisting Affiliates needs to be provided with all the information related to his/her medical conditions. The following form needs to be filled by the wish child's doctor.

Thanking Partners, Volunteers and all those people that helped granting a wish is crucial to have them engaged. To simplify this part we created a template of the Thank you Letter.

In this folder, you will find all of the documents that are related to specific celebrity wish request or general information on Celebrity wishes.

Some chapters from Make-A-Wish America receive a lot of wish assist requests and have therefore developed request forms to collect the required information. If your wish child is traveling to an American chapter territory, check this folder to see if they have created a request form for you to complete.

In this folder, you will find information sheets from Affiliates on some of the cities that are most regularly visited. We have also added a sample itinerary and the Wish Child Medical Travel Summary to this folder.
