The first step of the Wish Journey begins when the affiliates receive the application form. These forms can be submitted in a variety of ways. Some affiliates have a direct link on their website, others receive the forms or emails directly from the hospitals, and some referrals come in by mail.
The following documents are necessary steps in the referral process.
This is the form parents have to complete and it covers publicity authorization, liability release and medical disclosure authorization.
Before you can start the wish granting process, this form needs a parent’s signature. Please remind the parents that even if they do not approve the publicity authorization, this does not mean their child’s wish will not be accepted.
For affiliates based in the European Union, note that from May 2018, the rules on data protection will change; please check with your legal advisors on any changes that are required on the Parents Form. We are doing the same at the International Office and will update the EU affiliates on progress.

This Wish Authorization Form must be completed and signed by the referred child’s physician and returned to the affiliate. This is not required for wishes that are not considered risky or dangerous for the child or do not involve long travel. However, all Phase 1 affiliates are required to obtain this form from the child’s physician.
This is the form that must be completed when making a referral. It has all of the child, parents and physician’s details on it.
This form needs to be filled and signed from the child's physician before a travel wish and updated on the Portal as the Assisting Affiliate/Chapter needs all the information related to the wish child medical condition.
