The forms listed below are accessible through the provided link, but please read these steps carefully before using them to ensure you are working with a personal copy and not altering the shared files.
1. Log into your wisr.edu email account, and
2. go to the link, you should download a copy to your own computer--do not start typing information into the form found on the link, because then everyone else will get a form that is not blank but has your information in it.
3. For most files that are in Word Document Format, you should click on the "download" icon [see download icon symbol at the top of this page!]--that is located in the top right corner of the page, and download a copy of the file to computer as a Word Document, so you can use a blank copy of the file for each course, each time you need to do so.
However, for those files that immediately open as a Google Doc, click on "file" in the top left corner of the toolbar at the top of the page, and then select "download" and then select Word (docx) format--however if you don't have the Word program on your computer, select Open Document Format (odt), and follow the instructions in the next paragraph on how to get FREE open source software that uses odt (word-like program).
If you do not have a copy of MS Word on your computer, there is a FREE open source software that uses odt which is a word-like program that works very well. (to download the free, Apache Open Office, go to https://www.openoffice.org/download/ )
Note that the "log of collaboration" will open as a "Google Sheet" and for that you will need to click on "file" in the top left corner, and then when selecting download in either Microsoft Excel or Open Document Format.
4. Again, you should then save a copy of each form to a folder on your wisr.edu Google Drive and, if you wish, also in a folder in on your computer. It is recommended that you save it to a folder, titled “Forms_LMS_WISR” in your Google Drive—saving it on your computer is optional, and some of you who are not yet proficient in using Google Drive may wish to also save in a folder on your computer until you become proficient in working with Google Drive.

Note: about WISR’s transition to using Google Education Suite Apps for all WISR administrative and academic functions. At this time, the vast majority of WISR faculty and students are comfortable in using Google Drive, and all officially submitted paperwork is saved in the WISR Administrative Google Drive. Still we know that some of you may wish to use your computer in addition to, not instead of, using Google Drive. Remember, you should be saving drafts of all your work on Google Drive. Students have lost significant portions of their writing by a computer crash or virus—saving your work on Google Drive is important to preserve your work, and, in addition, to long-term ease and efficiency in using WISR’s Online Course System that is based in the Google Education Suite.

Note you must download YOUR OWN copy and will not be able to edit this link. After downloading add your first and last names to the title of the file, before “Log of Collaboration Revised November 2018”. Also, note that at the bottom there is a tab that provides a separate sheet for each course you take. Change the name of the Sheet to the number of your course (for example, from “Course 1” to “MFT 551” etc.) For this particular form, unlike the Word Document type forms, you should download a copy of the form by clicking on “file” in the top left corner and on the drop down menu, look for, “download as” and choose the file format you wish to use, most likely "excel" (or alternatively, Open Document Format).
