Independent Living Center of the Hudson Valley (ILCHV)

The Independent Living Center of the Hudson Valley provides services and support for people with disabilities, including youth, young adults, and family members.

The Independent Living Center has a Youth Transition Specialist that facilitates a weekly youth group. The youth group offers people, through age 30, recreational opportunities, skill building, art, music, and peer support.

Other services include assistance with housing, employment, education, referrals, and other identified needs.

Other Services Offered:

  • Open Doors- assistance and info for nursing home staff and nursing home residents who want to live in the community

  • Olmstead Housing Subsidy- HUD voucher and light case management for people who want to live in the community, who have been in a hospital/nursing home for 120 days or more in the last 12 months.

  • Youth Transition Specialist- Youth group for young adults 14-30 with any disabilities, assistance for families/students working with Special Ed, IEP meetings and annual reviews, transition plans, community resources, and peer support.

  • Benefits Advisor- assistance applying for social security/disability benefits, or appeals, special needs pooled trusts and more.

  • Medicaid Facilitated Enrollers (FE) - for seniors and individuals who are blind or disabled.

  • Consumer Directed Personal Aid Program (CDPAP)- Medicaid funded home aid service managed by individual, with payroll support from ILCHV.

  • Veterans CDPAP- Consumer Directed personal aid service for veterans, registered with the VA.

  • Wellness Programs- support for smoking cessation, specially designed gym for wheelchair users- open to the public, occasional workshops in self-defense, yoga, music and fine arts etc.

  • Systems Change- federal, state, regional, local advocacy for ADA compliance and issues related to supporting legislative and practical changes that support independence for people with disabilities.

  • Access to Home- grant for Medicaid recipients to build ramps and other environmental modifications.

  • NY Connects- Information, referrals, assistance with resources for individuals with disabilities, covering 19 counties.

  • Peer Support- individual counseling for people with disabilities by people with disabilities.
