Athlete & Coach Status

SOMN changed the medical process so that a physical is no-longer  required.  The medical will need to be renewed every year using the new form.  We are planning how we will handle the renewal process. 

The coaches can check their own status on the coach portal and review their Check List.

Check the status of your Special Olympics Minnesota (SOMN) requirements for Athletes and Coaches.  Blazing Stars Special Olympic Team Registration will be presented on each sport page as a roster for the current season -- no full athlete names nor full coach names will be listed. 

Explanation of  Information Displayed in the Status Table  Presented Above.
Only the 1st two letters of the last name are shown.   For example, Doe will be listed as Do.
First Name and Middle Name or Initial as submitted.   This depends on how registration was submitted to SOMN.
Medical Effective Date is the health care workers signature date from the last submitted physical.
Medical Expiration Date is when the current physical expires.  The value is RED if it needs to be renewed this year (2023).  If blank, the athlete registration is incomplete.
Consent Effective Date is the date that last consent was signed by the athlete's guardian or by the athlete if the athlete is the guardian.  If blank, the athlete registration is incomplete.
Consent after an athlete reaches 18 years of age generally does not expire unless guardianship changes.
Consent Expiration Date is the date the athlete reaches 18 years of age or when guardianship changes.  Blank is normal for most athletes age18 and older.

Certification identifies the name of the certification held by the coach.  Level 2 Certifications are not required for all coaches.  Each sports team requires 1 or more level 2 coaches.
Effective  date is the date the certification was added. 
If the effective date box is blank, it means that the initial certification is not complete.
Expires date identifies when a certification needs to be renewed.
If the date is red, the certification IS expired or will expire this year, and must be renewed.
Level 2 renewal is only required for the sport/activity identified.
