Physical Examinations

Physicals for 9-12th grade will be Thursday, Jan. 10th starting at 1:30 pm in the librarty. Physicals are $10. These physicals are for all players including those who have had their physical this summer. This will insure you are eligible for all summer play and activities. We will have another physical in August. I apologize for the late notice. We were not informed until this afternoon that they would be occurring tomorrow. If you are not in Soccer P.E., come by room to pick up a form if you do not have a printer, otherwise, print the form below. You will be called out of class during 8th Period. Please make sure your DragonFly has been updated. Fill out all information except your Physical Exam. We will upload that when you get your physical. Here is the information for DragonFly.
