The behavioral survey forms are to be used for obtaining behavioral data from households, community groups, and schools. These forms contain questions related to pathways of exposure to fecal contamination for those included in a specific SaniPath deployment. Forms are separated by the context in which the behavioral data is being collected, whether that be in an individual household, at a community meeting or gathering, or during a school class of children. Only minor changes should be made to the forms regarding specific exposure pathways that might not be included in all SaniPath deployments.
