We are involved in a pilot to enable patients to monitor blood pressures at home. Each practice in PCN1 will be given blood pressure machines to give to patients. The CEG have produced a search which can identify which patients to prioritise. This page contains guidance on the pilot programme.
Each practice should identify an admin and a clinical lead for the pilot
Read the information on this page and decide on the process for finding patients and how to give them the blood pressure machines.

Mark the BP machine and box with a number and name of the practice
Create a log book (found at the end of the above document)
Communicate with the practice team. We suggest you discuss this at your staff and clinical meeting. You will find suggested emails to send to practices on slide 16 of the presentation below.
Consider creating AccuRx text to share information. You will find suggested text for AccuRx on page 16 of the main presentation slide.
Run the CEG search (see below) and identify your patients
Contact patients and start the pilot programme. You will find suggested messages and website links for patients on slide 16 and 17.
