Case scenario: you call a patient for a routine follow-up. During the call you realise that they sound unwell, weak and they are just lying in bed. There is no-one to help her. This is clearly different to what they are normally like.
This patient needs urgent/emergency medical review.
First decide if they need 999. This is for emergencies that need a fast response. Eg breathless, chest pains, strokes. Don't worry about calling 999. They will guide you through the assessment and provide help.
If it can wait, call the practice duty doctor (sometimes called the on-call doctor). Use the bypass number to contact them. Inform them that you feel the patient is unwell and may need urgent medical advice. Explain that you are seeking their advice on what to do next.
If it can wait and you are unsure and cannot get through to the practice, discuss with the PCN team or clinical director for advice and guidance.

Always think, 'do I need to call 999'.
