Gravity Resisted Instructions: Ask the client to lie prone with hips and knees in 0° extension. The practitioner will stabilize the posterior hip and pelvis on the tested side. The practitioner will ask the client to "Raise your leg up toward the ceiling. Keep your knee straight." As the client extends the hip the practitioner will state "Don't let me push you down." The client will extend the hip, and the practitioner applies resistance to the posterior aspect of the midthigh in the direction of hip flexion.
Grades: Normal (5), Good (4), Fair (3)
Gravity Minimized: No resistance is applied, and the practitioner or the table supports the testing extremity. (Not pictured)
Instructions: Client is side-lying on the side not being tested. The lower extremity being tested is in 0° of hip extension, and the lower extremity not being tested is in 90° of hip and knee flexion. The practitioner will stand behind the client and support the extremity being tested. The practitioner will ask the client to "extend your hip and move your leg behind you." The practitioner will palpate the hip extensor muscles (gluteus maximus, semimembranosus, semitendinosus, and biceps femorus).
Grades: Poor (2), Trace (1), No contraction (0)
Gravity Resisted Instructions: Ask the client to sit in a chair or on the edge of the treatment table with legs hanging over the edge with hips and knees at 90° of flexion. The practitioner will stabilize the opposite hip and thigh and then ask the client to " Lift your thigh off the table as high as you can go." The client will move the hip into flexion while the practitioner applies resistance to the anterior middle thigh in the direction of hip extension.
The practitioner will ask the client to "bend your hip toward your chest." The practitioner will palpate the hip flexor muscles (Iliopsoas, sartorius, tensor fascia lata, and rectus femoris),
Grades: Poor (2), Trace (1), No contraction (0)
Gravity Resisted Instructions: The client is side-lying on the treatment table, with the hip and knee being tested in 0° of extension, and the lower extremity not being tested is slightly abducted and supported by the practitioner. The practitioner will stabilize the nontested leg and then ask the client to "Move your bottom leg up toward your other leg." The client will move the hip into adduction, and the practitioner applies resistance to the medial aspect of the thigh in the direction of abduction.
Grades: Normal (5), Good (4), Fair (3)
Gravity Minimized: No resistance is applied, and the practitioner or the table supports the testing extremity. (Not pictured)
Instructions: The client is supine on the treatment table, with the hips and knees in 0° extension, and the lower extremity being test is abducted. The practitioner will ask the client to "move your leg toward your other leg." The practitioner will palpate the hip adductor muscles (pectineus, adductor magnus, gracilis, adductor longus, and adductor brevis).
Grades: Poor (2), Trace (1), No contraction (0)
Gravity Resisted Instructions: The client is side-lying on the treatment table, with the hip and knee being tested in 0° of extension, and the lower extremity not being tested is flexed to 90°at the hip and knee. The practitioner will stabilize the hip and pelvis and then ask the client to "Keep your leg straight, raise your leg toward the ceiling." The client will move the hip into abduction, and the practitioner applies resistance to the lateral aspect of the thigh in the direction of adduction.
Grades: Normal (5), Good (4), Fair (3)
Gravity Minimized: No resistance is applied, and the practitioner or the table supports the testing extremity. (Not pictured)
Instructions: The client is supine on the treatment table, with the hips and knees in 0° extension, and the lower extremity being test is adducted. The practitioner will ask the client to "move your leg out away from your other leg. " The practitioner will palpate the hip abductor muscles (sartorius, gluteus medius, gluteus minimus, and tensor fascia latae).
Grades: Poor (2), Trace (1), No contraction (0)
Gravity Resisted Instructions: Ask the client to sit in a chair or on edge of the treatment table with legs hanging over the edge with hips and knees at 90° of flexion. The practitioner will stabilize the thigh and then ask the client to "Move your foot out to the side, while keeping your knee bent." The client will move the hip into internal rotation, and the practitioner applies resistance to the lateral aspect of the lower leg in the direction of external rotation.
Grades: Normal (5), Good (4), Fair (3)
Gravity Minimized: No resistance is applied, and the practitioner or the table supports the testing extremity. (Not pictured)
Instructions: Client is supine, with the extremity being tested in hip external rotation and 0° of knee extension. The practitioner will ask the client to "Point your toe towards your other foot and rotate your entire leg inward." The practitioner will palpate the hip internal rotator muscles (gluteus medius, gluteus minimus, and tensor fascia latae).
Grades: Poor (2), Trace (1), No contraction (0)
Gravity Resisted Instructions: Ask the client to sit in a chair or on edge of the treatment table with legs hanging over the edge with hips and knees at 90° of flexion. The practitioner will stabilize the thigh of side being tested and then ask the client to "Move your foot inward toward your other foot, keeping your knee bent." The client will move the hip into external rotation, and the practitioner applies resistance to the medial aspect of the lower leg in the direction of internal rotation.
Grades: Normal (5), Good (4), Fair (3)
Gravity Minimized: No resistance is applied, and the practitioner or the table supports the testing extremity. (Not pictured)
Instructions: Client is supine, with the extremity being tested in hip internal rotation and 0° of knee extension. The practitioner will ask the client to "Rotate your entire leg outward away from the other leg." The practitioner will palpate the hip external rotator muscles (piriformis, quadratus femoris, obturator internus, obturator externus, gemellus superior, and gemellus inferior).
Grades: Poor (2), Trace (1), No contraction (0)
