To print and write on the forms:

1. click on image

2. click the printer icon in upper Rt corner

3. print and fill out

4. mail to the committee chair

To type on the forms:

1. click on copy to Google drive link

2. make a copy

3. type on the form, making spacing adjustments

4. either SHARE or EMAIL the doc to the committee chair
