While the process is always customized to the goals, assets, and needs of each individual student and family, the Bridges re-entry process typically involves the following steps.
STEP 1: Re-entry meeting & intensive services (approximately four weeks)
Understand student’s clinical needs and identify needed mental health supports
Determine course-load/schedule (schedule time in Bridges, course-load changes)
Communicate with teachers (determine essential work/establish work plans)
Determine initial grading options if needed
Communicate with/set-up outside support
Daily check-in with Bridges support staff
Email communication to parents and support team from Bridges staff.
STEP 2: Four-week check-in with parent/guardian and school support staff.
Observations of student readiness to participate in school (arriving on time, utilization of Bridges Program services, classroom attendance)
Recommendations for future student program (course-load/schedule changes, outside support frequency/type)
Plan Bridges Program involvement moving forward (for next 4-5 weeks):
Weekly email to parents and support team
Communicate with outside support, as needed
Check in with Bridges support staff
STEP 3: Check-in with parent/guardian and school support staff (week eight)
Observations of student readiness to begin transitioning out of Bridges Program.
Recommendations for future student program (course-load/schedule changes, outside support frequency/type)
Initial discussion of program exit timeframe and process Bridges Program involvement moving forward:
Student drop-in as needed
Communicate with parents/guardians, as needed
Communicate with outside support, as needed
Check in with Bridges support staff, as needed
STEP 4: Bridges exit meeting with parent/guardian and school support staff
Data- and observation-based discussion of student readiness to exit Bridges
Recommendations for future student program (course-load/schedule changes), online classes, summer school, repeat a course in future year, outside support frequency/type)
Guidance counselor resumes case management
Clarification of “Bridges Alumnus” status: when student can visit the program, possibility of serving as support for other students engaged in Bridges currently and in the future, etc.
