Islamic Art Field Trip Registration - Saturday Feb 11, 2017

Sunday School students of age 11 and older can sign-up for the trip.
Adult chaperons will accompany students during this trip.
It is a day-long trip. Students and Chaperons will assemble at MCCGP and will leave after Fajr prayers.
A $50 fee per student and $20 per chaperone is due at the time of registration and registrations without payment will be considered void.
Payments can be made via Paypal "Register" link below. Please remember to fill out and submit the form before making the payment. Choose one of the applicable payment options.
Breakfast, lunch and Dinner is included.
We have limited seating and therefore will encourage everyone to sign-up as soon as possible.
If you'd like to register more than one child, please signup separately. You can choose to pay one time by selecting the appropriate option.
Registration is only open for students of designated age groups at this time.
If you are a parent who will like to volunteer as a chaperone and/or driver, please indicate as such at the time of registration (answer a few questions on the form).
