The GCS of a patient is the level on a scale of 3-15 of the PT’s consciousness. The GCS of a patient is determined by 3 factors. The PTs:
Opening of eyes
Verbal responses
Motor Responses

Determine the score of the PT's Eye-opening response (A)
Determine the score of the PT's Verbal Responses (B)
Determine the score of the PT’s Motor Responses (C)
Add A+B+C together and that is the total GCS score.

To pain: 2
None: 1
Verbal Responses:
Oriented: 5
Confused: 4
Inappropriate words: 3
Incomprehensible sounds: 2
None: 1
Motor Responses:
Obeys commands: 6
Purposeful movements (pain): 5
Withdraw (pain): 4
Flexion (pain): 3
Extension (pain): 2
