By authority of the Secretary of Health, signed and issued by Dr. Diana R. Tamondong-Lachica, Assistant Secretary of Health for Care Delivery, Quality and Patient Safety, Hospital Systems Cluster, and Concurrent Director IV of the Health Facility Development Bureau (HFDB); addressed to all DOH executives, central and regional directors, BARMM Ministry of Health, directors of specialty hospitals, chiefs of DOH hospitals; includes as annex to the issuance a copy of the draft revised HCES form, and a link to access the same, and provide comments for consideration therein.
This issuance originated from DOH HFDB: Address further inquiries about this issuance to the Bureau's Policy, Planning and Program Development Division.
