By authority of the Secretary of Health, signed and issued by Atty. Christian Lel Saquilabon, Assistant Secretary of Health, applies to all government hospitals and DOH Centers for Health Development, includes as annexes to the issuance, a copy of the revised HCES tool, and sample template of the confidentiality and non-disclosure agreement. Transitory provisions included for the implementation of the revised tool.
