From August 7 through September 15, 2025, the Cortland County Health Department, Guthrie Cortland Medical Center, and community partners collected feedback from 2,807 Cortland County residents through the Community Health Assessment (CHA) survey. The results will inform the 2026–2030 CHA/CHIP by:
Capturing community perspectives on healthcare access, major health concerns, and social factors affecting overall well-being, while comparing results to the 2019 and 2022 assessments.
Providing updated local data on chronic disease, mental health, substance use, and other emerging health issues.
Identifying disparities among population groups in Cortland County related to chronic disease, mental health, substance use, and other health and social challenges.
