From August 7 through September 15, 2025, the Cortland County Health Department, Guthrie Cortland Medical Center, and community partners collected feedback from 2,807 Community Members through the Community Health Assessment (CHA) survey. The results will inform the 2026–2030 CHA/CHIP by:
Capturing community perspectives on healthcare access, major health concerns, and social factors affecting overall well-being, while comparing results to the 2019 and 2022 assessments.
Providing updated local data on chronic disease, mental health, substance use, and other emerging health issues.
Identifying disparities among population groups in Cortland County related to chronic disease, mental health, substance use, and other health and social challenges.
From February-April 2022, the Cortland County Health Department, Guthrie Cortland Medical Center, and partnering agencies collected input from 2,266 members of the Cortland County community. The survey was utilized for the 2022-2024 CHA/CHIP to:
Determine community member perceptions related to healthcare access, health problems, and social determinants of health and track trends in community perceptions compared to the 2019 Community Health Assessment Survey.
Obtain more recent data related to chronic disease, mental health, and substance use in the Cortland County community.
Identify disparities in sub-populations in Cortland County related to chronic disease, mental health, and substance use.
