For trainees seeking to complete fellowships in both Advanced Heart Failure (AHF) and Critical Care Medicine (CCM), each a one-year ACGME-accredited program, the challenge of securing positions in both specialties at the same institution can create uncertainty. Additionally, relocating for a single year of training can present logistical difficulties.
To address this, the University of Minnesota Division of Cardiovascular Medicine is proud to uniquely offer a dual-track fellowship pathway, providing guaranteed admission to both programs at the time of acceptance. This pathway ensures a seamless transition between fellowships, offering continuity in training and mentorship while allowing fellows to maximize their expertise in both subspecialties.
Fellows accepted to this pathway will:
apply to the AHF fellowship, and if matched, transition directly into the CCM fellowship the following year, contingent on maintaining good standing in the program;
train in a consistent, collaborative environment with faculty dedicated to both subspecialties; and
have flexibility in structuring their fellowship sequence, though we strongly recommend completing the procedurally intensive fellowship (CCM) second to facilitate a smooth transition into clinical practice.
Upon completion of both fellowships trainees will be board eligible in both Advanced Heart Failure and Critical Care Medicine.

The AHF Program Administrator will coordinate with applicants and the CCM Program Administrator to arrange a joint interview day. Candidates who match into the AHF fellowship through this pathway will automatically secure a position in the CCM fellowship program for the following academic year contingent on maintaining good standing in the program.
If a fellow wishes to complete the CCM fellowship first, they should apply to the UMN CCM fellowship through ERAS and indicate interest by sending a message to the AHF and CCM Program Administrators.
