Did you have an injury in camp that required more than a Band-Aid? Fill out the form using the Accident/ Close Call / Coach Injury Event Type
Everything else to do with injuries, such as Band-Aids, ice bags, camper sitting out for a while, report using the minor injury report section
Any behaviour or discipline issues use the form to let admin team know. We cant help if we don't know about it.
