As you work with your students, ayou might need advice or help in managing your class beyond the usual academic issues. We encourage you to seek the advice of the Chairs Office and to report all incidences. The Chairs Office will track the incident and assist you in finding help and consultation, if needed. We appreciate your assistance in maintaining the classrooms. Please enforce the following rules:
No food or drink in the classrooms.
Replace all furniture removed from rooms or rearranged in the room before class ends.
Remove trash and belongings when class is over. Any belongings left in the rooms will be thrown away the following morning.
Please sweep the floor if necessary. Brooms are available in each room.
Do not move rehearsal furniture or chairs from other rooms.
The Chairs Office is the Department office. The Chairs Office and its serves as the faculty member leading administrative functions in the areas of personnel, curricular guidance and student affairs. If you have any questions or issues, please contact the Chair directly. Should there be an issue with student conduct in your classroom, we encourage to you to contact the Chairs Office as soon as possible.

Plagiarism

Center for Accessible Education (CAE) is a campus resource that assists the faculty and the University in meeting academic accommodations mandated by Federal and State law and University policy. Professors are legally responsible to provide the students with equal access to education. If a student inquires about or seems to require services, please refer them to CAE. CAE evaluates the student’s documentation to determine how the disability impairs the student’s academic performance and then will provide services and accommodations to compensate for the documented disability. Faculty who do not get specific instructions from CAE should not accommodate a student’s request.

Indications that a referral and/or consultation call should be made:
Physically aggressive acts or intimidating behavior
Domestic violence or sexual assault
Harassment or stalking
Implying or making a direct threat of harm
Willful or intentional behavior causing damage to property
Direct of implied threat of suicide or death
Weapons in the UCLA community
Life-endangering substance abuse, eating disorders or other high risk behaviors
The Title IX/Sexual Harassment Prevention Office provides information about campus policies and procedures to any interested person. Individual consultations can be arranged for persons who need detailed information about possible sexual harassment and options for resolving concerns on campus. Individual consultations are also available for faculty members, supervisors, managers, administrators and others who need information about their responsibilities to address information about possible sexual harassment of which they may become aware.

Below is an example of wording used in previous syallbi regarding Title IX information. Please include this information in your syllabus.
Title IX prohibits gender discrimination, including sexual harassment, domestic and dating violence, sexual assault, and stalking. If you have experienced sexual harassment or sexual violence, you can receive confidential support and advocacy at the CARE Advocacy Office for Sexual and Gender-Based Violence, 1st Floor Wooden Center West, CARE advocate@caps.ucla.edu, (310) 206-2465. In addition, Counseling and Psychological Services (CAPS) provides confidential counseling to all students and can be reached 24/7 at (310) 825-0768. You can also report sexual violence or sexual harassment directly to the University’s Title IX Coordinator, 2241 Murphy Hall, titleix@conet.ucla.edu,(310) 206-3417. Reports to law enforcement can be made to UCPD at (310) 825-1491.
Faculty and TAs are required under the UC Policy on Sexual Violence and Sexual Harassment to inform the Title IX Coordinator should they become aware that you or any other student has experienced sexual violence or sexual harassment.
