Requesting a Transcript for University Applications

Adult students (21+)  applying to university are required to request that their transcripts are sent to the Ontario University Application Centre  (OUAC) at the end of each quad (if a new credit was earned).

BEFORE you make your request, please find the following information:

• Legal Name (Last, First)

• OEN Number (this can be found on your Student Status Sheet, Transcript,) 

• TDSB Student Number # (this can be found on your uploaded student ID, timetable or transcript)

• Date of Birth 

• OUAC Application Number

If you want an official transcript (signed, embossed and sealed) you will need to come to the school with photo ID to get it in a sealed envelope and you mail it yourself.

All students studying in Toronto have an Ontario Student Transcript (OST). Transcripts are records of student achievement and are required for post-secondary institutions, employers and for a number of other purposes. All school boards in Ontario are mandated to maintain the Ontario Student Record (OSR) folder including a copy of the Ontario Student Transcript (OST) for 55 years after the student is demitted. Transcripts are available to all former students on request and for a fee.'

Please speak with reception in Room 119 OR send your request by email to cityadultstudentservices@tdsb.on.ca and include

• Full Legal Name

• Date of Birth

• Student Number (optional)

• If you have been out of school for 2 or more years:

  • • please request your transcript for the Central Transcript Office.
