Physical/Medical Information

Physicals for student-athletes are offered by the school district for no cost at various times throughout the school year.  The school nurse keeps a list of student-athletes who have had physicals, and this list will be shared with coaches.

If a student-athlete is not on this list, he or she may NOT practice or participate in a game.  The student-athlete must present his or her physical paperwork to the school nurse, who will verify the physical and, if approved, will enter the student-athlete into the shared list.

Each coach in the Southern Tioga School District must maintain emergency information on all student-athletes who participate on the team.  This information must be kept confidential and must be in the possession of the coach at all athletic functions (practice, game, transportation, etc.). Use the ATHLETIC INSURANCE / EMERGENCY FORM to collect/maintain this information.
