If you need to move an initial transition visit, please notify Patient Access Transitions team via email, PAsingletransitions@solacehealthcare.com. Email should include the patient's name, MRN, orginal visit time, and the new requested time. Please also enter a communication note that you have confirmed this new date/time with the caregiver!
