If a client is taking 5+ meds, clinicians must ensure the POC is correct.
In the MatrixCare assessment note, "5 Meds" is NOT an option.
In order to ensure accuracy in documentation and compliance, a clinician MUST select the option marked "Other".
Any clinician who enters a medication update needs to then add in a miscellaneous type communication note to the chart, informing the team of the change.
The Case Manager must include a MEDICATION STATEMENT in the "Interventions" box of the Additional Notes section in the Pediatric POC:
At the time of evaluation include "Reviewed medications with family at SOC." and
With each 60-day ongoing POC, reflecting on the whole 60-day period, not just the current visit or previous week.

It is the responsibility of the Case Manager to verify that the Medication Interaction Statement is included in the POC Document. If the statement does not pull through, it needs to be entered into the "Interventions Notes" box prior to submitting the POC.
If you cannot fit an entire Major/Severe medication interaction into the "Interventions" box of your note due to the character limit, please:
Include the following statement: "Major/Severe interaction noted. Due to character limit, interactions included within the Recert Notes Section"
You may then copy/paste the full interaction(s) into the Notes section on the sidebar of the same page.
Remember, you only need to include new medication interactions
