Make sure that you clearly communicate discharge plans with parents/caregivers and with other care team members to make discharge a green dot experience for everyone.

See above for a sample discharge email, including final parent education and access to the Family Feedback Form.  Remember- you have a relationship with the families you serve.  Please feel free to modify this template to your style as it is just a recommendation!

At your final visit and in your discharge email, be sure to remind families that we are here for them if they need services in the future!  If they are still receiving services from another discipline, that clinician can fill out a Universal Referral Form.  If not, they can always request a referral to Solace from their physician.
