When do I need to complete additional documentation related to a hospitalization?
Additional documentation needs to be completed when a client has been hospitalized >24 hours.
Documentation should be completed within 24 hours of the clinician being notified of the hospitalization.
How many clinicians in the home need to complete this documentation?
Only one clinician in the home needs to complete the Agency Transfer document in Clinical Documents
However, all clinicians will need to perform a re-evaluation (Pediatric POC or FUVN) at their first visit back in the home following hospitalization. The first clinician to perform a visit will complete a Pediatric POC note and all others will complete a FUVN.
Is there anything special that I need to know related to our Therapy Assistant Program?
Yes- Thank you for remembering our Therapy Assistants! Since Therapy Assistants are not able to complete assessments, they will need their supervisor to complete a supervisory visit either before they return to the home or at the assistant's first session following a hospitalization >24 hours. It is extremely important to communicate hospitalizations with the entire care team to allow appropriate arrangements to be made!
