If you are the first clinician in the home, admission documents will be sent to you via DocuSign from a Care Coordinator in the Patient Access department and are specific to each client.
A day or so before your initial visit, open the packet in DocuSign to sign your portion. Leave the packet in editing status by clicking "sign later" & then on the day of the actual visit fully "confirm signing" as the date in the packet must match the date the evaluation/first session took place. So, if you get started, but wait to fully confirm signing, then the packet is able to be modified/edited and you will not need to request a new one if the date of the eval changes last minute.
There are "Welcome Videos" the caregivers can watch to cover the key highlights of what is being signed in the packet. This video will be sent with the Admission Documents for telemedicine evaluations and via the Skedulo text reminder for in person evaluations. Please encourage the family to watch!
The PDF copies of Admit docs are always available to you in Google Drive as well & can be used as backup in the event that something happens with DocuSign. They are located in the Admission Documents folder in the Clinician Resources Shared Drive. Click the packet, then "open in" or "share to" acrobat adobe, click "fill & sign" to edit the packet. You do not need to sign into an account if prompted, look for the small faded "x' in the top right to bypass that pop-up if you get it. Please forward the signed packet to the Health Information Team at HIM@solacehealthcare.com so they can process and add to the clients chart.
DocuSign is the easiest way & you are prompted to sign/fill where needed. Please only use the PDF/Adobe as back-up.

There are two different processes to complete Admission Documents, depending on if your evaluation is in person or via telehealth.
For in person evaluations: Admission documents will be sent out as “in person signer”. This means that caregivers will now be able to sign admission documents with the clinician, in person, during the initial evaluation. Clinicians this means you can “host a signing” of the admission documents during your initial evaluation with the family!
Please note, with this version, the admit docs are not sent to the parent's email. Clinicians are responsible for obtaining signatures on their iPad, during the in person evaluation.
For telehealth evaluations: These will continue to be sent out as “remote signer”, meaning that caregivers will be able to access and sign the admission documents virtually, prior or during the initial evaluation.
If for any reason you need to change the signature method for an initial evaluation, please respond directly to the initial evaluation email that you received from the Patient Access team.
