Cancer Patient Services
The organization provides transportation and other services to cancer patients in the Mount Forest area.
There is no fee to cancer patients for any of our services. Patients who require support for transportation or other supports via CPS that they must meet either one or the other of the following geographic conditions:
Have a doctor or nurse practitioner (NP) as their primary health care professional and live within the boundaries of:
NORTH Durham
SOUTH Arthur
EAST Keldon
WEST Clifford and Harriston
-OR -
Live in the Mount Forest area and be within the boundaries of:
NORTH Southgate Road 14 (north of Holstein)
SOUTH Kenilworth Side Road (Wellington North Side Road 7)
EAST County Road 16 (east of Conn and Cedarville)
WEST Minto Road 3N (west of Pike Lake)
The area also includes Holstein, Letterbreen, Cedarville, Conn, Kenilworth and Pike Lake.
Should anyone who works with your outreach team have an individual that does not meet these geographic requirements and need transportation support for treatment they can instead utilize the Canadian Cancer Society’s transportation program called Wheels of Hope. It is important to note though that there is an initial one-time start up fee of $100 for new patients to use this service.

Patients who have used CPS previously can contact CPS directly to arrange transportation rides, however, if they have not accessed this service previously, they should be directed first to the cancer care coordinator Mishele 519-323-0255 x5014.
Transportation for Medical Appointment- VON
Transportation for ODSP, pre-approved medical appointments are covered for.
Also provides transportation for seniors but at a cost.
