Request Documents

Oasis is happy to provide both current and former tutors with letters of recommendation, tutor performance evaluations, resume reference forms, etc. It is our expectation that when a tutor wants our endorsement services, she/he fills out the form below so that we can personalize each recommendation.

Please note: It is not possible for us to offer last minute / emergency / off hours endorsement services.

Step 1: Fill out the Endorsement Form below.

Step 2: Send us an email with details of your request, including the individual or organization receiving the endorsement, the deadline, and any other special instructions. 

**Both steps required!**

Please allow us up to two weeks to complete your recommendation letter or evaluation, and note that these services are only available during our regular office hours when tutoring is in session. We will email recommendations or evaluations directly to an official contact person when required, but we do not email them to students. If you need a hard copy, please email us and we will send it by mail. If you have any questions, feel free to email us!

If you want an official copy of your hours report, fill out the following form. Keep in mind that you can only request a copy of your official hours report to be sent to professors and organizations through email. Tutors may request a hardcopy.

*Students registered under a professor do not need to request for the monthly report to be sent, we do that automatically.
