1.6 Status of maternal death control management system or program in terms of absence or presence; if present, when started; whether there is a team that meet regularly at planned intervals; whether there is a document that contains the program or management system; whether the program is comprehensive; whether there is regular tracking of implementation; whether there is a regular review of the program and continual improvement.
2.
