1.6 Status of maternal death control management system or program in terms of absence or presence; if present, when started; whether there is a team that meet regularly at planned intervals; whether there is a document that contains the program or management system; whether the program is comprehensive; whether there is regular tracking of implementation; whether there is a regular review of the program and continual improvement.

2. Offer an operational definition of “maternal death control management system” in the following setting: Hospital; Birthing Home; Rural Health Unit; Community Health Program Management Office (provincial health office; city health office; municipal health office; and non-governmental institution with a health care management program).
