What is the Maternal Death Control Management System Team?
It is a group of people identified by top management to work collaboratively as a team and authorized to establish and develop, implement and evaluate a Maternal Death Control Management System or Program. A Maternal Death Control Management System or Program is a set of interrelated and interfacing policies and procedures established and developed to control maternal death, that is, to control pregnant women from dying as a result of pregnancy.
What is the authorized functions of the Maternal Death Control Management System Team?
1. To strategize on how to control maternal death in the area of work or responsibility.
2. To formulate policies and procedures in controlling maternal death.
3. To deploy the design and development blueprint in controlling maternal death.
4. To supervise the implementation of the blueprint in controlling maternal death.
5. To evaluate and review the implementation outcomes of the blueprint and then formulate a continual improvement action plan.
6. To formulate and implement a documentation and an archiving plans.
7. To formulate and implement a management review and an independent audit plans.
Who should be appointed members of the Maternal Death Control Management System Team?
Guided by the authorized functions of the team, the members should at least be the following:
For hospitals and medical centers -
A representative from the Obstetric-Gynecology Department as head of the Team
A representative from the Anesthesiology Department
A nurse from the Emergency Department
A nurse from the Labor - Delivery Room or Department
A nurse from the OB Ward or Floor
A representative from the Laboratory Department
A representative from the Pharmacy Department
A representative from the Medical Records Department
A representative from the Finance Department
A representative from the Social Service Department
For birthing homes -
A representative Obstetric-Gynecology as head of team
A representative nurse
A representative mid-wife
For RHUs -
The MHO as head of the team

Who should oversee or supervise the Maternal Death Control Management Team in a hospital . medical center setting?
The Chief of Clinics or the Medical Director or the Hospital Director or the Hospital Administrator depending on the set-up
The overseer should ensure that the Maternal Death Control Management Team is functioning as planned and as authorized.
What are the responsibilities of the members of the Maternal Death Control Management System Team?
All should collaboratively work as a team to accomplish the authorized functions of the Maternal Death Contol Management Team.

OB-gyn:
Team leader - people management and resource management (competencies expected: leadership; managership; teamship; and technical aspect of work)
OB-gyn - ensures quality and safe medical care of pregnant patients
Anesthesiologist: (competencies expected: leadership; managership; teamship; and technical aspect of work)
Ensures quality and safe anesthesia care of pregnant patients
Nurse from Emergency Department: (competencies expected: leadership; managership; teamship; and technical aspect of work)
Nurse - ensures quality and safe nursing care of pregnant patients in the Emergency Department
Patient-need Coordinator - coordinator of medical and non- medical needs of pregnant patients in Emergency Department
Nurse from Labor - Delivery Room or Department (competencies expected: leadership; managership; teamship; and technical aspect of work)
Nurse - ensures quality and safe nursing care of patients in the Labor - Delivery Room or Department
Patient-need Coordinator - coordinator of medical and non- medical needs of pregnant patients in the Labor-Delivery Room or Department
Nurse from OB Ward or Floor (competencies expected: leadership; managership; teamship; and technical aspect of work)
Nurse - ensures quality and safe nursing care of patients in the OB Ward or Floor
Patient-need Coordinator - coordinator of medical and non- medical needs of pregnant patients in the Labor-Delivery Room or Department
Representative from Laboratory (competencies expected: leadership; managership; teamship; and technical aspect of work)
Ensures quality and safe laboratory services to pregnant patients.
Ensures availability of blood for pregnant patients
Ensures safe blood product administration to pregnant patients
Representative from Pharmacy (competencies expected: leadership; managership; teamship; and technical aspect of work)
Ensures availability of drugs needed by pregnant patients
Representative from Medical Records Department (competencies expected: leadership; managership; teamship; and technical aspect of work)
Ensures complete and accurate recording of pregnant patients and their needed data.
Representative from Finance Department (competencies expected: leadership; managership; teamship; and technical aspect of work)
Ensures complete and accurate accounting and budgeting of logistical needs of pregnant patients.
Ensures complete and accurate accounting of PhilHealth benefits of pregnant patients.
Representative from Social Service Department (competencies expected: leadership; managership; teamship; and technical aspect of work)
Ensures assistance of indigent pregnant patients.
How should meeting be held by the Maternal Death Control Management System Team?
Meets regularly every week, two weeks until all design and development blueprint has been completed. Thereafter, meets every month.
There shall be communication on meetings - date, time, venue, and agenda.

There shall be minutes of all meetings which are safely kept and archived for at least 3 years.
How should be the Management Review and Independent Audit be done?
The Maternal Death Control Management System shall be subjected to a management review by the top management every month initially, then every 2 months, then every 3 months, then every 6 months (once stabilized).
The Maternal Death Control Management System shall be subjected to an independent audit every year, then every 2 years, then every 3 years.
What are the key performance indicators of the Maternal Death Control Management System?
There is a document that prescribes and describes the organization of the Maternal Death Control Management System.
The Team meets regularly as evidenced by minutes.
The Team is fulfilling its authorized functions as evidenced by minutes.
The Team is conducting regularly an audit on maternal adverse / sentinel events and near-miss.
The Team is successful in achieving its targets, particularly on zero mortality rate.
