Policy:
The Maternal Death Control Management System shall be subjected to independent audits at planned intervals (at least once a year for the first three years then every 2-3 years thereafter).
Procedure:
The independent audits shall be done by the corporate internal auditor of the medical center or by the internal quality and performance auditors not involved in the Maternal Death Control Management System at least once a year for the first three years then every 2-3 years thereafter.
The internal quality and performance auditors shall not audit his / her own unit's program.
External independent audit on the Maternal Death Control Management System shall be done at least once a year for the first three years then every 2-3 years thereafter.
The independent audit shall be guided by the authorized functions of the Maternal Death Control Management System Team; the goals and objectives of the Maternal Death Control Management System; the operational plans and targets of the Maternal Death Control Management System; and other criteria like the presence of approach, deployment, learning, and integration for the processes and level, trends and bench-marking for the results.
The independent audit shall use document review, interview, observation, and tracer methodology.
There shall be documented action plans on continual improvement at the end of each independent audit.
