Recent LODD or near LODD events -

- 2007 - Boston MA - 1 firefighter left his radio back at the fire station

- 2009 - Houston TX - 1 fire officer left his radio in the fire truck - the other firefighter on on the wrong channel

- 2009 - Buffalo NY - 1 firefighter had radio turned off - 1 officer was on 'direct' instead of 'repeater'

- 2009 - Prince Georges MD - MAYDAY on direct channel - dispatcher gave wrong MAYDAY location

- 2010 - Bridgeport CT - confusion over MAYDAY call

- 2011 - Washington DC - multiple units on wrong radio channels
