RIASSUNTO: Il fenomeno "dolore" si può descrivere in cinque fasi funzionali:
1. Ricezione (eccitazione del nocicettore);
2. Trasmissione delle "informazioni nocicettive" dal nocicettore al talamo;
3. Distribuzione delle informazioni nocicettive dal talamo alle tre aree del cervello (sensoriale, cognitiva ed emotiva);
4. Integrazione delle informazioni pervenute al cervello;
5. Percezione del dolore
Il fenomeno dolore può essere considerato come costituito da cinque fasi funzionali:
1. Ricezione. Una sostanza lesiva per i tessuti (bruciante, schiacciante, tagliente, ecc.) viene a contatto con questi. Esistono nei nostri tessuti (cute, mucose, muscoli) miliardi di speciali formazioni istologiche, che hanno la proprietà di entrare in eccitazione se vengono in contatto con una causa potenzialmente lesiva per i tessuti; vere sentinelle in guardia a difesa della incolumità dei tessuti del nostro organismo. Queste speciali formazioni, dette nocicettori, (da noxa, agente lesivo) sono ad elevata soglia di eccitazione e questo fa si che si eccitino solo per cause lesive (in questo si differenziano dai recettori per il tatto).
2. Trasmissione. Allorchè un nocicettore viene eccitato in esso si forma un potenziale elettrico che lascia la cellula e si propaga subito come un’onda, lungo la fibra nervosa, penetra nel midollo spinale e risale fino al talamo (formazione ovoidale sita alla base del cervello). Queste onde rappresentano messaggio in codice, e sono dette “informazioni nocicettive” proprio che informano il cervello che i tessuti sono aggrediti da un agente nocivo.
3. Distribuzione. Dal talamo l’onda elettrica entra nel cervello e si distribuisce in tre aree: segnatamente sensoriale, cognitiva e emotiva.
4.- Integrazione. Le informazioni nocicettive si mescolano integrandosi lungo le vie nervose della coscienza e danno origine alla percezione.
5.- Percezione. L’individuo assume consapevolezza di cause lesive tissutali. Se aboliamo lo stato cosciente inducendo sonno (come avviene farmacologicamente in anestesia generale) non si ha percezione e l’individuo non avverte dolore ossia non assume coscienza della aggressione.
Modulazione. Durante il viaggio lungo il midollo spinale la informazione nocicettiva viene modulata (ridotta di potenza ) due volte. Una prima volta al suo ingresso nel midollo spinale. Si tratta di un circuito di modulazione descritto alla metà degli anni sessanta da due ricercatori: l’inglese Patrick Wall ed il canadese Ronald Melzack e battezzato dagli stessi con il suggestivo nome di “cancello all’ingresso”.
La patologia del dolore assume quindi tre significati diagnosi differenti, ciascuno dei quali richiede differente terapia.
Il dolore che insorge per alterazione della fase di nocicezione dicesi dolore nocicettivo, e richiede terapia con aspirina o aspirina simili.
Il dolore che insorge per alterazione della trasmissione chiamasi dolore neuropatico e richiede terapia con morfina ed altri oppiacei.
In fine il dolore che insorge per alterazione della fase di percezione chiamasi dolore psicogenico, e come terapia richiede l’impiego di farmaci sedativi e antidepressivi.

SUMMARY: The phenomenon of "pain" may be described in five functional phases:
1. Reception (stimulation of the nociceptor);
2. Transmission of the "nociceptive information" from the nociceptor to the thalamus;
3. Distribution of the nociceptive information from the thalamus to the three areas of the brain (sensorial, cognitive, emotive);
4. Integration of the information arriving in the brain;
5. Perception of pain
The phenomenon of pain may be considered as consisting of five functional phases:
1. Reception: Existing in our tissues (skin, mucous, muscles) are billions of special histological formations, which have the property of becoming excited if put in contact with something potentially dangerous for the tissues, real and true sentinels guarding the safety of the tissues of our organism. These special formations, called “nociceptors” (from noxa, harmful agent), have a high excitation threshold, and this means that they are excited only by harmful causes (in this, they are different from the receptors for touch);
2. Transmission: When a nociceptor is excited, an electric potential forms, which leaves the cell, and is spread immediately like a wave along the nerve fiber, penetrating into the spinal cord, and rising up to the thalamus (an oval formation at the base of the brain). These waves represent coded messages, and are called “nociceptive information” because they inform the brain that the tissues are being attacked by a nociceptive agent;
3. Distribution: From the thalamus the electric wave enters the brain, and is distributed in three areas: sensorial, cognitive and emotive;
4. Integration: The nociceptive information is integrated along the nerve pathways of the conscience, and give rise to perception.
5. Perception: The individual achieves awareness of the causes of tissue damage. If we abolish the conscious state inducing sleep (as happens pharmacologically in general anesthesia), s/he does not perceive, and does not feel pain, that is, s/he does not gain awareness of the aggression.
Modulation. During the journey along the spinal cord, the nociceptive information is modulated (reduced in Potenza) two times. The first time occurs when it enters the spinal cord, and is a circuit of modulation described in the mid 1960s by two researchers, the Englishman Patrick Wall and the Canadian Ronald Melzack, who called it with the evocative name, “gate.”
The pathology of pain, therefore, assumes three significant different diagnosis, each of which require a different therapy.
The kind of pain that arises for an alteration in the nociception phase, called nociceptive pain, requires therapy with aspirin, or aspirin-like substances.
The kind of pain that arises for an alteration of transmission, called neuropathic pain, requires therapy with morphine and other opiates.
Finally, the kind of pain arising during the phase of perception, called psychogenic pain, requires therapy using sedatives and anti-depressants.
