1. Patient Identification
Accurate patient identification is the foundation of clinical safety.
According to the Joint Commission (TJC) and the 2025 National Patient Safety Goals (NPSG.01.01.01), sonographers must utilize a two-identifier standard to prevent errors.
The Two-Identifier Standard
At every stage of the encounter, the sonographer must match the procedure to the individual using at least two distinct identifiers.
Protocol Requirements:
Active Verification: Do not ask passive questions (e.g., "Are you John Smith?"). Require the patient to state their full name and date of birth while verifying against the ID band and requisition.
Non-communicative Patients: If the patient is impaired or comatose, use a legal guardian, a second clinician witness, or barcode scanning.
Neonatal Protocols: Newborns must have distinct naming systems (e.g., "Mother’s Last Name, Mother’s First Name, Gender"). Identification bands should be placed on two different body sites.
