Procedure on Certification/Authentication/Verification from CHED

1. The student files the Requisition Slip with the Office of the Registrar together with the original copies of the documents to be authenticated. The Transcript of Records, Diplomas, and Related Learning Experience (RLE) is to be authenticated at the Registrar’s office.

2. Check the authenticity of the records (compare photocopy with the original documents or student’s permanent record).

3. Pay the corresponding fees to the finance office.

4. Keep the official receipt which should be presented in claiming the Certification on the scheduled date of release indicated on the form.

5. The Registrar certifies the authenticity of the documents and forwards the same through CHEDRO IV-A through the staff of the Registrar using the 1st endorsement form, or the applicant may bring the documents (sealed in an envelope) personally to CHEDRO-IV A if with proof of urgency.

6. CHEDRO- IV A verifies and acts on the certification submitted by the Registrar/School Liaison Officer after payment of the processing fee.

7. The Registrar’s Office secures the claim stub from CHEDRO – IV A and, once available (usually after 2 to 3 weeks), immediately gives it to the student who requested for CAV.

8. Requirements

· Clear duplicate copies of credential/s

· Clear duplicate of diploma

· Certificate of enrollment (for undergraduate only)

· Letter of authorization to request documents, in case students cannot transact personally.

For Individual applications (with proof of urgency) – within 7 working days

In highly exceptional cases, the applicant may be allowed to hand carry the said documents in a sealed envelope to CHEDRO IV. The applicant must submit a copy of proof of such urgent cases: flight details, visa application schedule, proof of deadline of submission, and letter from the agency, subject to evaluation. Starting August 1, 2022, only applicants with valid proof of urgency will be accepted.
