GUIDELINES FOR THE REQUEST OF TRANSCRIPT OF RECORDS (UNDERGRADUATE)


The release of records is from school to school. If the transcript of records has already been sent to another school and received by the latter, the party concerned requesting another copy should be directed to the school to which it was forwarded. If the requesting party whose record has already been sent to another school applied for another copy for the purpose of studies abroad, another copy may be issued only upon presentation of a certification from the school attended that it has no objection to the issuance of another copy to be marked “valid only for studies abroad.”

In case the request has mark “entrust to the bearer”, the Official Transcript of Records (OTR) is sealed in an envelope and bears the Registrar’s signature across the flap.
