10.1 Transition Assessment.

Format: PDF file & Website

Description: This two-page worksheet provides instructions for taking the Casey Life Skills "Education: High School" assessment. It directs students to copy a screenshot of their assessment results and answer three open-ended questions.

Instructions: Open the PDF file . Create an account so you have access to a Casey Life Skills account. Complete the assessment using your birthdate and email address from the worksheet. Select the "Education: High School" assessment from the drop-down menu. Once the online assessment is completed, copy a screenshot of the colored graph displaying your assessment results and paste it into the worksheet and then answer the questions. Once completed, the PDF file should be added to your student file.

10.2 High School Goals.

Format: PDF file

Description: This two-page worksheet contains checkbox-style and short-answer questions about your short-term goals in high school.

Instructions: Open the PDF file. Once completed, the PDF file should be added to your student file.

10.3 Study Skills.

Format: PDF file

Description: This three-page activity directs you to identify how often you uses a variety of study skills using a rating scale. Use these results to determine your study strategy "skill level". The activity concludes with two brief survey questions.

Instructions: Open the PDF file. Identify how often you use each strategy on page two by marking the square, triangle or circle for each (using the key at top). Follow the directions on page three and use the skill "calculator" to calculate your score. Once completed, the PDF file should be added to your student file.

Description: This two-page worksheet contains several scenarios and related short-answer questions where students would have an opportunity to use self-advocacy skills.
