This developmental curriculum provides a framework for transition activities for high school students. Each student will complete a minimum of four activities every year in each of the following domains:

• Assessment
• Goal Setting & Planning
• Academic & Career Skills
• Self-Determination

Click on the appropriate grade level page above to reveal the transition skill focus areas for that grade level. Activities can be downloaded directly by clicking the activity link. Detailed instructions are also provided with each link. Students should also complete a variety of assessments on the "Transition Assessment Page" to add to their record file.

After completing each activity the files should be stored in the student's record folder. Keep a record of you activities and progress by printing and completing the "Transition Checklist" after every IEP meeting.
