Transition Planning is a process of preparing students with a disabilities to move from school to adult life.

This website is designed to help students and family members navigate the transition planning process. Student's with disabilities often face unique challenges as they prepare for life after school. Understanding the options, resources, and supports that are available can be overwhelming.

In Massachusetts Public Schools, the transition planning process begins when a student turns 14. As the student gets closer to graduation, their IEP Team will consider how to prepare them for a successful adult life. Considerations include:

1. Job Training.

2. Post Secondary Education.

3. Employment.

4. Independent Living.

Just as every student's abilities and interests are unique, every student's transition plan will be different. This site includes resources that will help with the transition planning process and answer many of the questions that families and student's commonly ask.
