Transcripts: A copy of the student’s most recent transcript. For new 9th grade enrollment coming from outside SAUSD, please provide the last report card from the forwarding middle school.
Withdrawal grades from most recent school, if student is enrolling during the school year.
Immunization records: Students must have all required immunizations.
Residency verification documents: Per AR 5111.1 (a) Provide current copies of ONE of the following documents (dated within the past 60 days), showing a current address in the relevant attendance area:
(a) Utility Service Contract, Statement, or Payment Receipt (e.g., water, gas, electric, or waste management);
(b) Telephone Bill (landline or cellular bill);
(c) Mortgage/Rental Contract, Lease, or Payment Receipt;
(d) Property Tax Bill or Payment Receipt past year;
(e) Pay Stub; (f) Voter Registration;
(g) Correspondence from a Government Agency;
(h) Car Registration (dated within the past year);
(i) Car Insurance Statement (dated within the past year);
(j) Bank Statement;
(k) Federal or State Income Tax Forms (dated within the past year); or
(l) Declaration from Primary Resident/Owner indicating that student is living with Primary Resident/Owner. If none of the above documents are submitted, the parent/guardian is required to execute an Affidavit of Residency.
Birth Certificate/Proof of Birth : Birth Certificate is needed for Verification purposes, for spelling of name and date of birth
We do not accept disconnect notices or mobile phone bills.
Please inform the Counseling Office staff if the registering student is in foster care.
Please inform Counseling Office is registering student has an IEP or 504 Plan. If so please see additional forms needed below.
Once you have completed the steps outlined, please contact the Counseling Office to schedule an appointment to submit your enrollment documents.
Special Education documents: Special Education staff members will review documents prior to enrollment to ensure appropriate placement of the student.
a) IEP: Provide a copy of the most recent IEP.
b) Psychological report: Provide a copy of the most recent psychological report.
Section 504 Plan: Please provide a current copy of your child’s Section 504 Plan.
Language Assessment (LPAC)
Expulsion Documents (if currently expelled from a public school or private school)
Custody Orders or Restraining Orders: Please provide any current order from the court before the start of each school year or when there is are changes.
Medical Orders: Please provide any current medical orders or other district medical forms upon enrollment and before the start of each school year or when there are changes.
Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses
(4 doses OK if one was given on or after 4th birthday. 3 doses OK if one was given on or after 7th birthday.)
For 7th-12th graders, at least 1 dose of pertussis-containing vaccine is required on or after 7th birthday.
Polio (OPV or IPV) — 4 doses
(3 doses OK if one was given on or after 4th birthday)
Hepatitis B — 3 doses
(not required for 7th grade entry)
Measles, Mumps, and Rubella (MMR) — 2 doses
(Both given on or after 1st birthday)
Varicella (Chickenpox) — 2 doses
California schools are required to check immunization records for all new student admissions at TK/Kindergarten through 12th grade and all students advancing to 7th grade before entry. Parents must show their child's Immunization Record as proof of immunization.
Students who live outside of Santa Ana High School's boundaries, but within the boundaries of a different SAUSD high school, you must obtain an Intra-DistrictTransfer Form to attend Santa Ana High School.

The district rule is that all students attend their “home” school of residence. An Intra District Transfer request is given to a parent wanting their child to attend another SAUSD school. You must obtain the form, and choose the “special program” option offered on the Transfer Form.
You will then meet with the principals of both the sending and receiving schools. The principal of the sending school (school of residence) and the principal of the receiving school sign the permit acknowledging that they met with the parents and student regarding the request to transfer schools. If for some reason both principals are not available, the parents are to make an appointment with the Senior Director of Pupil Support Services (PSS), or Assistant Principal of Guidance.
This Inter-district transfer agreement is valid only for the school year granted; the agreement expires at the end of each school year and must be renewed annually.
This agreement may be REVOKED at any time by the district of attendance for the following reasons: -
Student is excessively tardy or absent from school, or student is brought to school excessively early or left excessively late.
Student fails to uphold appropriate behavior standards.
Student has poor academic performance.
Insufficient space in the school and/or grade level.
False or misleading information was provided.
Students or parents fail to follow school rules.
Students entering grades 11 and 12 in the subsequent school year shall not have their agreements rescinded by either district. Approval is subject to space availability in the district and may not be at the site requested.
If the student participates in any athletic program governed by the California Interscholastic Federation (CIF), he/she may not be eligible to participate at the new school. Parent/guardian should check the CIF rules before submitting this agreement.
Students who are eligible for Special Education Services may be asked to obtain an Inter/Intra-SELPA Agreement for Individuals with Exceptional Needs, in addition to the Inter-district transfer agreement. No financial obligation shall be incurred by the district of residence for services rendered under this agreement. The parent/guardian is responsible for providing transportation to and from school.
