The IEP must be amended when…
The student has met their goal
There needs to be a change in service times or location
There needs to be a change in accommodations or adaptations
There needs to be a change in special considerations
There needs to be a change in LRE
An IEP can be amended with or without a formal full team meeting with parent agreement. In both scenarios, parents must be informed about the proposed changes via a Written Notice and IEP Amendment. Below is a screenshot of the steps to complete an IEP Amendment in Edplan.

Use drop down> Choose Amendment
Identify date of Amendment (IEP Begin and End Date remain the same).
Fill in the details about why the IEP is being amended, and select the sections of the IEP that will be amended.
If you are creating an Amendment without a meeting, you must also create a Parent/Adult Student and District Agreement to be sent with Amendment - this document can be created from the link next to Amendment Details within the IEP Amendment process and is also found under Documents in blue bar.
To add or change services: Always start as of the date of amendment. Old service ends the date before the amendment. NEVER delete data from IEP, always end and start new. ADD new service lines, don't delete information about PRIOR services.
Embedded Written Notice: The original statement “Annual IEP Completed” carried over from annual - add amendment date and explain amendment. Continually add new amendment dates and statements. NEVER DELETE data in IEP.
Finalize the IEP Amendment. If parents agreed to the proposed changes without a meeting, a Parent/Adult Student and District Agreement form (above) must be sent home for a parent signature with the finalized IEP Amendment. Make note in contact log.
The date requested within this agreement regarding the IEP is always the end date of the current IEP (amendments must always be within the date of current IEP).
IMPORTANT REMINDER! NEVER DELETE SERVICES! Service changes are noted by stopping old services and starting new services. Old services must be ended the day before the IEP Amendment and new services started the day of the IEP Amendment. The amended services always end the date the annual (actual) IEP ends.
Year End Amendments
For service changes due to grade transitions (5th to 6th, etc.) - old services end on the last day of school and new services start the very next day. Remember the IEP is telling a complete story and IEP Amendments must be noted--we do not delete details.
If you are adding narratives within the IEP (e.g., Optional Statement of Service Delivery, Written Notice) - start with the date of the IEP Amendment and then add your information. This will make it easy for a reader to understand what has transpired over the course of the year with the IEP.
Example: Optional Statement of Service Delivery
3/7/2020: Additional academic support to provide assistance with task completion and concept reinforcement will be provided by the special education teacher or paraprofessional under the supervision of the special education teacher in the special education resource classroom for 240 minutes per week.
Finalized/Amended IEP and Parent/Adult Student and District Agreement form are sent home to parents for signature. The parent and a school administrator must sign the Agreement form.
Scan and upload the returned/signed Parent/Adult Student and District Agreement form to the student's EDPlan record as an attachment to the finalized IEP Amendment so that it is associated with that event.
As with annual IEPs, the IEP Amendment is also to be finalized within 5 days of the meeting or team decision being made (if no meeting is held).
